why would a cat not go through a cat door even when she has to go to the bathroom?

Cats abhor water, so putting a cat door in the bathroom door is pretty pointless. Get her a litter tray.
I'm assuming you mean a cat flap so that your cat can go outside? Has she ever used it? If not, maybe she doesn't know how!

I had to 'train' my cat to use his cat flap, firstly by holding it open and tempting him through by waving food from the other side. Then when he was used to hopping through with it open, I closed it and waved food/treats at him. He soon got used to the idea and uses it all the time now!
