Im trying to find a particular poem about Hugs?

A few years ago i heard a lovely poem about hugs, i dont remember the title or author but i do remember a bit of it
'when im not there with you and i cant hug you when i would just follow these directions to feel somewhere near as good, keep this card where your reminded that my arms are seeking you and when you feel you need a hug, this is all you have to do.....'
