Tramadol tends to have 'away with the fairies' side-effects for many people, sqad? I hated the stuff and was much happier with codeine/paracetamol based pain relief for my knee.

do you actually mean effervescent or do you just mean liquid? Is it because she can't swallow tablets? as many can be crushed and mixed with jam, honey or yoghurt. If the tablets she is taking now are effervescent, it may be the fizz that is making her stomach sore...I can't do fizzy type tablets at all.
I have been taking Co-Codamol 30/500 for several years. It does cause constipation, but if that is what your sister is bothered with then the doctor could prescribe something to alleviate that. If it is definitely a stomach upset and not constipation, then please make sure that she takes the tablets with food, this should stop any stomach upsets.
