I was at the Dr last week and she wants me to do a swab and take it in, but there is nowhere to put my name and she didn't give me a label. So would it be ok to write my name on a sticky label and presumably put my date of birth on it and notify reception when I take it - would this be ok and would I need to include any other information? ie Patient number?

Take it in and get them to put a label on it while you're there.
Name, date of birth and date of test, with "swab from....".
Let them do it. They must be geared up for that sort of thing. If you had the swab taken at the clinic, the nurse would do this as a matter of routine.

it wasn't done on site but they normally give you a label (she is a new dr so maybe she wasn't aware and I didn't think about it at the time)
