Can anybody please give me some useful weblinks for information about the care and support somebody can expect to receive once they've been discharged from hospital following a stroke, and what kind of help (if any) is available from Social Services (in term of finance and equipment /house adaptation. Thanks

The NHS website will give you the answers you are looking for just put Stroke aftercare and support in the search facility. Community care arrangements should be part of the discharge procedure at the hospital.They send someone out to the patients home to assess the patients needs, equipment etc.
Community provision varies enormously area to area. The patient's Occupational therapist and Social Worker (where appointed) should provide this information.
