Period pain

I have always had exceptionally heavy periods, from being 11 to now at 45...i was told after having babies it would get better...no chance! I was sterilised when I was 25 (due to having a chronic kidney disease) I was told I might start the menopause early. My periods are not regular at the moment, always seem to be anything from 5-15 day late each time!
My problem is that for quite some time now I have the period and then maybe a week after finishing I am getting some pretty horrible period pain again...maybe for another week...no blood just the pain, why so soon after finishing a period?? Could it be fibroids?? My Mum had them I remember.
