The alkaline phophatase is an enzyme in all cells of the body, but particularly in high concentrations in the liver and bone and any disease process which attacks these two organs will enable the enzyme to "spew" out into the blood stream.
In the liver one thinks about gallstones and in the bones, osteoporosis.
Also certain medications, particularly containing female hormones can raise the alkaline phophatase levels.
