What personal health information do I have to disclose to my employer? i have been given a questionnaire to fill in and am concerned in case it is used as evidence against me, rather than in a supportive role.

If you are employed and it comes to light that you have fibbed on such a questionnaire, then trust has been broken between you and your employer, they are entitled to sack you and you have no redress.
