personal health information for employers

What personal health information do I have to disclose to my employer? i have been given a questionnaire to fill in and am concerned in case it is used as evidence against me, rather than in a supportive role.

I'm afraid that you really have to fill in the questionnaire truthfully. Or say that you prefer not to answer.
If you are employed and it comes to light that you have fibbed on such a questionnaire, then trust has been broken between you and your employer, they are entitled to sack you and you have no redress.
Sorry, but I'm pretty sure that's the situation.
