Sadly, this is the kind of survey that could upset a vulnerable person. There's no evidence that taking vitamin supplements has any greater effect than painting the back door green. Ditto with water consumption in 'glasses of water' not counting the water content of other food and drinks. The choices of response are directed toward forcing you into a description that may not be accurate. For me. the 'something to think about' is the design of useful electronic surveys, and the ultimate purpose to which the information is put.
