Yes, Prep H. or failing that, if VERY severe, surgery.
They very rarely "go away" but it is common for them to "cure " themselves.
If they become prolapsed, ie come out of your "bottom" they become very painful, but if they become "thrombosed" i.e strangulated, then you are in big trouble.

To start with, take the advice of janbee above for 24 hours.
My haemerroids got so bad in my late 20s that I had to have two operations to have them cut and tied.
It truly is no joke but if you think the pain is bad with them, there are no words to describe the agony I was in for two weeks after the first operation.
The only comfort I could recommend is what is called a Sitz bath.
You just fill your bath with four or five inches of the hottest water (not boiling) you can bear, sit down and grit your teeth.
All the creams in the world wouldn't get rid of my piles but maybe like you, they may be past that stage now.
It seems there must be different sorts. I've had a permanent one that isn't painful for years. An have occasionally experienced a short term painful one. GPs never seem interested in offering to get rid of the former, I don't know why.
I don't know that GPs themselves deal with them, but they ought to be able to refer you on? Mine did.

That is the attitude with piles.
I'm sure at the last prostate examination the locum called the former, a skin flap. Was too surprised to ask why, as no GP previously had, and the fact there was a student there threw me more. Maybe it isn't vital but not something I really wished to have over the last, what, twenty years ? It'd be nice to have it removed but not fussed enough to pursue it.

But DD has my sympathy, the painful ones are ... well a pain in the *** whilst they are there.

Think they cauterize them these days rather than cutting and tying!
until you seek medical advice. ask pharmacist for anusol with HC. should give you some relief.
Mine went away, I had piles from sitting on cold benches in spain waiting for buses and trains while travelling through spain, (yes you can get them from sitting on cold surfaces for a long time) First time I ever had them, bloody and messy but not really painful, they went after a couple of weeks. that was 10 years ago, that was the one and only time I have had them.
Dieseldick.......I thought that he post was self explanatory........sometimes they do go away "remain symptomless" and sometimes they don't.

Is that clear?
I had them when I was pregnant. I went to the GP and he said "oh just push them back in"!!! NOT what I expected to hear.

You have my deepest sympathy dick, I had mine banded many years ago, extremely uncomfortable for about a week afterwards, but very little trouble since.

When the first appeared I went to see my GP.
He asked me in his gentle Irish brogue, 'Have I seen these piles before Mr Garador?'
'No' says I, 'You haven't Doctor'
'Well, I'd better have a look, hop up onto the bed.' 'Mmmm, I've one thing to say about them.'
'Whats that Doc?'
'I'm blooming glad they're not mine!'

I went to the doctors about mine and he referred me to hospital. The doctor there said they were skin tags and not piles, anyhow he booked me in for surgery and I had them removed last month. The relief is bliss, knowing I can sit down and not have someone stick a red hot poker up my a**e at some unsuspected moment, at least that's what it felt like.

I had suffered for years and tried all the creams, with no success. So my advice would be get to the doctors and have them removed. Embarrasing, yes, but well worth it, if you ask me.
