When you get signed off by the doctor do you get an actual piece of paper these days ...... I know you can self certificate for four days but wasn't sure what happens for a longer period

i was off for 5 weeks with a fractured foot. every week i had to call my doctors surgery and say what days I was off work. They filled out a piece of paper with the dates on it and i kept it for my accountant. I had to do this every monday morning until i was able to go back to work.
self certificate is for the first seven days , if you are off work after that your gp will give you a sick line for however long he decides which is called a Med 3 form which you fill in your details on the reverse eg national insurance num, payroll number
SSP is around �70.00 per week

meant to say, as long as you have seen the doctor and they have diagnosed what is wrong with you then they will fill out the certificates
I used a self - cert for the first five days and then when i went back to the A&E outpatients two fridays ago they gave me a 4 week sick note, which runs out 23rd September. I sent it into work immediately and I cannot return to work until the expiry date on the sick note, unless i pay my doctor a fee to dclare me fit for work and override the previous sick note.
