Perhaps the nurse or doctor found the patient deceased upon entering the room, and could not give a precise time since no one actually witnessed it.
Yinzer, I was going to post the same. My father died in his sleep (merciful release). He was being checked 2 hourly and was found dead

We went in the hospital to visit my dad and we found him dead in a bed on the ward.  The doctors and nurses didn't have a clue until we told them.

thanks for replying, the doctors were there when he died.

but no time of death was recorded.as it is the first death

certificate i have seen

i thought a time had to be recorded by law.

When my sister died [in hospital] the time of death was given as the time the doctor signed the certificate, even though there was a 2 hour difference.  Can`t say why no time was given in your example though.
Then maybe they just forgot?
