bob...I was only a GP for 3years and not a very good one at that....hated it.
Well...
a) If no transport was available...relatives usually had good excuses for not being available.
b) Collapse(for whatever reason), severe breathlessness or cardiac pain.
c) Immobility due to trauma.
d) If they were "mouthy" like some ABers, I would tell them to get to hospital under the their own steam.
Usually, the Receptionist was excellent and she organised that end of the practice.
