How do we go about changing the name on the house deeds?

My father in law is ill and would like to sign the house over to us while he is still able to do so.
There is no mortgage to pay.

And it doesn't prevent the use of the assets of the individual to pay for long-term care home costs, should that be the intended purpose of the transaction.
