risk retention examples

Hey, got a test on monday for retention and transfer of risk, and I thought it would be a good idea rather than just to get the theory but to find real life examples, for example a company that has a captive is BP whom own Jupitar Insurance LTD.
After a few searches there are some examples I cant get my hands on, does anyone know where i can find such examples as companies which have faced unplanned retention due to ignorence(BP oil spill???), unplanned retention due to confusion of contractual terms, examples of transferring risks to a non-insurer, and perhaps an example of when a company has financed its loss with non-replacement?
