if you go back to being employed from self employed, what form do you fill out instead of the P45? thanks

You don't need to fill out any forms, just inform the tax office, submit your self assessment when due and that's all you need to do.

If you havent got a P45 when you start a new job you^r employer will give you a P46 to fill in.
