if your company is not vat registered do you still have to show the vat element on your cash book?

You mean on the purchase ledger, I suppose.
There's no particular reason to do so since the VAT cannot be offset anywhere.

You don't have to.

Some people choose to do so and have a VAT expense heading but most don't.
