Contact the Health Professions Council, on 020 7582 0866 .
Susan M Cooper is (as she's legally obliged to be) registered with the HPC. (Registration number HAD00294). They should know where she is but (unless she's indicated that her home address can be given out for business purposes) they won't be able to tell you directly.
However, if you express your concerns over your current problem, the HPC may well contact Ms Cooper directly, and insist that she contacts you. (As the HPC has the power to 'strike off' anyone whose conduct is regarded as unprofessional, it's unlikely that she'd fail to comply with such a request).

However it's possible that the HPC might consider that your report relates only to 'consumer service'. which they don't regulate. Even so, all is not yet lost because Susan Cooper is also a member of the British Society of Hearing Aid Audiologists, who have their own complaints procedure:

Hi,
I am in the same position, I purchased a hearing form Chobham hearing in Jan 2006 and took it in to be repaired April/May 2010. The shop was closed for several months due to bereavment and illness according to the note on the window. Then the shop was completely closed down later last year. I left messages on the answer phone but no response. I have found an address on line in Farnborough under company reports. I haven't contacted it myself yet, but hope it may help.
