Leaving the catholic church

After deep soul searching,I find that I wish to leave the Catholic church,Is there any formal declaration I need to make,and if so to whom ? I do not wish to be considered 'lapsed',but to return to Anglicanism.

If you were originally baptised as an Anglican, I'm pretty certain that baptism will still be valid. There is no formal renunciation from Catholicism. You don't get a decree nisi, or anything like that. Just return to the church of your choice. If you weren't baptised in the Anglican church and want to receive communion there, your local vicar can arrange for your to be confirmed into the church.
