The world revolves around them, and them alone, and people in general, are nothing but tools, to do or get things for them.
and as vicalncraig says, in the early years, its very hard to spot, and even when they are older, unless there is something else wrong, to other people, they look normal, so their behaviour is just put down to misbehaving.
Probably a bit more than you wanted, but I tend to get carried away with this subject.
