Are you asking about whether to inform the doctor that you couldn't pull the skin back? It is my understanding that the foreskin doesn't fully retract until 5 or 6, so I wouldn't worry about that. If it worries you, check with a doctor to ease your mind.
If you are asking about checking with a doctor because he touches himself... that's totally normal and nothing to worry about.
It's just something that little boys do, he'll soon grow out of it. It will be difficult to retract the foreskin at that age anyway.
If by some quirk of fate he persists and does retract it DO NOT attempt to roll it back into place, as this can cause a condition called paraphomosis. Seek the advice of your GP, though as I said this is a difficult thing anatomically for a child to do at that age.
I would still strongly recommend you ask the doctor to look. I had a similar problem which was ignored (even though my dad had to be circumcised for the same problem) & I was eventually circumcised - at age 19! No regrets at all about having it done but I had many years of discomfort because no-one did anything about it when I was a kid.
It so happens that my nephew (who we were caring for when he was the same age as your boy) seemed to have the same problem. We took him to the doctor who at first was totally dismissive but I insisted he checked properly. Within seconds he realised that things weren't quite right & referred him to a specialist.
There is no way any of us can tell if your little boy has a problem or not - make an appointment with the doc.
