i spend half my time in Norfolk and half in suffolk. i had all my mw appts in norfolk and with me first went to hospital in norfolk and then stayed at my mums there for first 2 weeks.
with my second i had all appts at norfolk including hospital appts, i then gave birth rather quickly in Suffolk and was discharged to my home there where i saw HV and MW. They did get a lot of notes mixed up but it wasnt too bad.

I agree it depends on how far you're moving. If you are willing to travel however then you could just not tell your midwife that you are moving. This should be fine as long as you don't need a home visit & are having a hospital birth. Unfortunatly if you do need a home visit then she will probably refer you to someone else as she's unlikely to be willing to travel.
Good luck. I know how you feel as I moved almost exactly a month before I gave birth. xx
