...and was refused entry because of ill health, how did he/she get home? The boat that brought them might have already left.

They got sent to England for free treatment under the NHS:)
I presume they were put on the next available boat back.

Here's an interesting article about Ellis Island...with information on what happened to those who were ill...not a quick turnaround.
Family lore says my Italian grandfather made the trip eight times before settling and having a business as a tailor.
I presume he worked,earned money,took it back to Italy...and supported his relatives.
Not unlike today.
