However Cain goes on to marry a wife? So either that was either his sister or the population of the earth was more than just Adam, Eve and Cain.

Adam and Eve were the very first people. There were no others. So Cain had to marry his sister, Because it was God's plan to populate the earth, the problems of birth defects caused by sibling procreating did not arise. All of them then were much closer to perfection then, and we now are the furthest from perfection. It explains heart problems and cancer.
