The phrase 'cold war' was coined by George Orwell in 1945. Very soon afterwards, it was used to describe the state of hostility between Russia and all the western countries...not just the USA.
A 'hot war' would obviously be one in which actual fighting was taking place with bullets flying etc, whereas a 'cold war' is one in which there are many threats, lots of propaganda and so on, but no actual physical conflict.
