I've never heard of this before, doesn't sound like any of theusual problems such as clubroot ot a fungal infestation, and I've been growing toms for over 30 years - Is it in a draft at all? - It could just be a plant defect, sometimes it happens - I had a plant a few years back that had every leaf split in half, the fruits were small and yet it was treated exactly the same as all the others and they did fine - just one of things maybe. Sorry I can't be of more help.
likely to be a spontaneous mutation...if it produces fruit it is likely being less protected by the leaves they may ripen better.... if this is the case save some seed and see if it is reproducible over a few generations...might be a commercially useful ' problem'
