Buttonholes are usually made up of a single flower, such as a rose or a carnation, with occasionally a little foliage, such as ivy leaf or fern.  Mens buttonholes are usually that simple.  Ladies buttonholes may have a little more adornment - another bud or two - perhaps a feather, but anything more elaborate and it would become a corsage.
