I have had two titles failed to transfer because the solicitor didnt understand the nature of the title - it wasnt freehold but equitable and that completely defeated him.
The other title the solicitor failed to transfer I had the assets recovery agency ask me to prove it was not an under-market transfer with an intent to defraud creditors
and a third one where A and B owned a property jointly, I bought out B and wanted my name in B's place ...... and the solicitor transferred A's share into B's name who became owner of the whole part. Luckily B had a sense of humour, and the transfer was unwound.
I only use licensed conveyancers now
