When I get an app invite from a friend has that friend actually sent it or is it done randomly. Getting invites to play games from some friends but don't want to ask them personally.

I don't think they are just sent randomly but some of the games seem to deliberately make it difficult to tell when you are sending a request to other game players and a request to all friends. They (your friends) won't be told if you refuse the request and you can block being sent requests from that game if it gets annoying.
