After the water goes down your sink (or elsewhere) it empties into ever increasing sizes of pipes which collect the water from other houses. It empties into a rather large drain in the middle of your street. This drain, depending on the size of the town or city is probably about 18 to 24 inches in diameter. Eventually, it either flows or is pumped to a sewage treatment plant where solids and the occasional diaper, etc., are removed. The waste water is then sent to a settling pond or connected series of ponds called lagoons. Bacteria breaks down any remaining solids and the water is is then evaporated naturally and/or moved into a river or ocean. The water, at that point is environmentally safe, if all the systems are working properly... This description has been simplified, but gives you an idea of how the system works...
Here is a simple diagram showing what happens when sewage (waste water) leaves your house. sewagetreatment
