What form do you fill in to change the address in your passport? Or who do you send your passport to?

go to the post office. they will have all the forms you need, and will be able to tell you what to fill in
Your passport has nothing to do with your address. The only reason you have to give your address is so they can send it to you. If you look in your passport, you will see there is no address on it. You can write in 2 addresses of people to contact in an emergency, but as you write these in yourself, you can tippex them out and change them yourself.
As sddsddean says, the addresses at the back are for you to manage. If youw antt o change then, just past over a piece of paper and write on the new one. If just that if you get ill abroad there's a contact point.
