Proper ones work by converting hard calcium carbonate into sodium carbonate via an ion exchange system. The sodium ions come from sodium chloride (common salt) which gets consumed in the process. Sodium carbonate (like all sodium compounds) is soluble in water. Calcium carbonate is limescale.
