Not 100 % on this but in UK hospitals we have SHO's (senior house officers) Consultants and registrars. The consultant is the highest, their title is Mr or Mrs, etc and they are usually surgeons as well. SHO's are under the consultants and usually see the less extreme cases, also helping out in surgery. Registrars generally have some speciality in their field of medicine and can do day sugery.  Not sure how helpful that was, i was just a receptionist in a hospital once.

I'm sure someone with "inside knowledge" will give a clear explanation but, from my many years of hospittal work, I remember it at this (from junior to senior):

House Officer.

Senior House Officer

Registrar

Senior Registrar.

Consultant.

In the UK, a Registrar who becomes a surgeon (Fellow of the Royal College of Surgeons -FRCS) is referred to as "Mr", not Dr".  hence, surgical and consultant surgeons are "Mr".

Physicians of the same rank are "Dr".
