I am interested in travel insurance with a difference, namely, essentially medical and repatriation only and on a multi trip basis. I realise this may be called medical insurance rather than travel insurance. In any case, I wonder if anyone can suggest an insurer for this - I have in mind taking on the other risks myself of baggage, bookings, etc., etc.
Yeah, but most of the financial risk associated with providing these policies is surely going to be in the medical and repatriation bit anyway? 'Loss of a camera', by comparison, ends up being chicken-feed.
So, given the fixed administration costs in setting-up and managing a policy, I don't see how this is going to save much, if at all.

Having phoned them it seems to me that the policy will remain recognisably a travel one but the flexibility consists of a number of add-ons to beef up each of the three main basic blocks of the policy. In fact, my main reservation before even speaking to anyone at M&S was the fact, very visible on the site, that AXA are the underwriters. In the past I had a policy with Guardian Royal Exchange, maintained over decades including some time after AXA took GRE over. AXA gradually whittled at the cover and the policy was becoming very different. Then I had a claim and the process they put me through was humiliating to the point that I vowed never to have any further dealings with them, even though they eventually (grudgingly ?) paid the claim in full.
Don`t forget you need personal liability insurance as well. If you were in the US (for example) and you accidently injure someone, they`ll sue you to high hell. I`m with Insure and Go (a higher level one) but their Bronze service is more basic - for example there is zero baggage cover but you have the important elements. It`s only about £45 which isn`t worth quibbling about really.
