PVG Disclosure

hey, i am filling out a disclosure form and its asking for my id number and i dont no where to find it, help? x

So, you have ticked yes to the question "
Are you now, or have you ever been a member of the PVG Scheme"?
Then you must have an PVG ID. At a guess it is more than likely at the top of your disclosure but you will need to read it thoroughly to find it

i havent answered it yet but i have a old disclosure from before the name got changed to PVG, my old one has a 15 digit number at the top.. but the form is asking or a 16 digit code. :s x

If the old scheme wasn't called PVG then you haven't ever been a member, have you? :-)
