right place for this i hope, what is a jsa4 form for (jobseekers allowance) friend has been told she needs to fill one in and should have done this last time she was at the jobcentre but they didnt tell her until she has checked to see if any money went into her bank and hadto ring to find out why it hadnt (efficient arent they!!!)

It doesn't ask so many questions as on the full claim form and the information from the the previous claim is used in conjunction with that on the JSA4. The previous claim must have ended for whatever reason but a letter should have been sent out explaining why it had terminated.
yes both answers make sense as she lost her claim for a month as they said the reason she left her job wasnt good enough reason but she has still been signing on and they should have got her to fill the jsa4 in and didnt so they are sending one out will ring her and tell her this many thanx to you both x
If someone leaves a job without a good reason then their benefit can be sanctioned. That means there is no benefit payable for the period of the sanction but that does not mean that the claim has ended, it carries on and the person carries on signing but gets no benefit. The JSA4 is a new claim but it sounds like the form that should be filled in is a JSA3. It's a form that is used to report a change of circumstances but it's also used in cases such as the OP's pal when a sanction has ended and it confirms that the circumstances have NOT changed.
