social, what has the person have in way of social activity's?
physical, what are the persons physical needs, if any ?
intulectual, how are they intulectually challenged every day?
cultural, what are this persons cultural needs/ background
emotional, needs?
spiritual, needs?
you then prepare a care plan using the s.p.i.c.e.s
risk assesments are then drawn together, if this person had mobility issues, how would you minimise risk when out and about ? whilst still promoting choice and their safety? the risk assesment then is intergrated in to the care plan. everyone who supports this person will work by the stated care plan and basic core values. ensuring safe working practices are being upheld, and the level of care is consistent.
a wheelchair for transportation. pick an accessable route/ destination. most are wheelchair friendly, due to the disabillity discrimination act 2006.
