Letters of Administration for Probate - what Affidavit is required?

My mother passed away very recently and left no will. Therefore I need to apply to be Administrator of her Estate. I have found all the relevant docuements on the HMCS website however i am required to swear an Affidavit... but can't find any detaisl as to what this shoudl say. It is simply "I'm her daughter, my address is , no will has been found can i please be administartor" If anyopne knows where I can find a form of words that would be very helpful.

When my Mum died without a will, the probate helpline sent me all the forms and when I had completed them I sent them to my nearest probate office. They then asked me to go for an interview and I had to swear that all information was correct and then they sent me letters of administration about a week/10 days later. I imagine this is what you will have to do. it may seem daunting but I found it relatively easy.
Swearing the affidavit is simply a matter of placing your hand on a Bible and reading what's on the card which you'll be given. (If, like me, you're an atheist - or any other form of 'non-Christian', simply ask to 'affirm' instead of 'swear'. You'll just be handed a different card to read from, without the use of the Bible).
