copyright permission approval for art

Someone whom i sold a sketch to wishes to get the sketch put into canvas form in ASDA, however they are refusing to let her do this as they need a headed letter from the artist giving permission. What should I say in this letter? I have no idea how to lay it out to give approval for my work to be reproduced? Anyone know a website that has a template i could follow?

This should do:

"To whom it may concern:

I confirm that I am the artist who produced the sketch illustration of St Maud's Church, which is now in the possession of the purchaser, Mrs Ethel Higgins.

I further confirm that I retain the copyright in the said work.

I hereby grant Mrs Ethel Higgins the right to have a canvas reproduction of the sketch made by Asda, and indemnify Asda against any claim for copyright infringement which may be made in respect of such reproduction.

Signed: Janet Pearson, 02/08/11"

I suppose you could say
"To whom it may concern, I drew the sketch ****, and sold it to Mrs. YYYY. I have no objection to her transferring this sketch onto canvas for personal use." I'm no expert but to me, that would give her permission for a single copy, but means she can't reproduce it many times and sell on the copies.

As usual Chris is there when we need him.

Two points; I would respectfully question whether 'confirm' is the best word and would offer 'I, Janet Pearson, am the artist...' and also I would enumerate how many reproductions were being allowed.

scotman is right on mentioning permission for just the one copy....or else she may reproduce copies for her family, Uncle Tom Cobbley and all.
