What you have said, the Dozer was reversing, It should have had a reversing Buzzer, a banks man, interior mirror for the reverse maneuver, depending on the dozer & size, was the operator qualified to operate that machine? did they have regular H/S/ tests? was the dozer up to standards? the Jack hammer operator should have had ear protectors on "Hence" the need for a banksman & High alumina jacket on. I think this will be the same old story regards H/S/ It's ok until something happens?

Surely using AB and getting peoples knowledge and ideas is doing research. No different from googling any other site in my opinion and Sandra can learn from it. I can't see the problem.

The guy on the drill should have been working in a safe designated area within the site. The bulldozer should never have been allowed to enter that area. Poor site and H&S management...yes the company are to blame.
