If someone is charged and sentenced with GBH and further down the road the victim dies, having never recovered from their injuries, can further charges be brought?

Yes. The original charge can be upgraded from GBH to murder, and the case reviewed.

I think there is a time limit though between the attack and death of the victim - sure I read it was one year and one day.
