Can anybody tell me that if money saved in a bank in a joint account - does it need to be declared if one of the jointers (old lady) goes into nursing care. Thanks

No. You both still have the account. You may want to change things later. As far as the bank is concerned one of you is temporarily living at a new address. But they don't even have to know. Things get a little more complicated in regard to Council Tax and, potentialy, with HMRC. Suggest honesty is the best policy. No-one's done anything wrong.
Declared when the lady’s assets are assessed for payment of fees? Yes I think so.

the joint account is £5,000 between the old lady and my cousin. I did think the old lady was allowed £16,000 savings
Yes it must be declared. But only half of it is deemed to hers for these purposes.
Depends on what else she has really doesn't it. House, shares etc
Not sure what the current rules are on how much allowed but everything has to be declared and then the allowed amount is deducted.
ALL assets must be declared, shared or otherwise
