Restriction on property

I want to register a restriction against someones property. They have completed a 2nd charge document to my benefit, but due to mortgage arrears the 1st charge holder will not consent to it. What form do I use? Land Registry won't give me the answer, as they say it constitutes legal advice. I think it might be a UN1 form, but can anyone clarify. If it is a UN1, can I complete it myself, or must a solicitor do it?
