With regard to the form LPA PA - could anyone tell me where I can find the notes relevant to filling in the form please?

Reading through the form, there is reference to these notes ... eg "see note 42"" etc. but I'm unable to access them from the main page on line.

The notes are in pdf format, so you'll need Acrobat Reader on your PC. You've almost certainly already got it but, if not, you can download it from here:
http://www.adobe.com/products/acrobat/readstep 2_allversions.html

There are separate notes for a 'property & affairs' LPA and for a 'personal welfare' LPA.

The property & affairs notes are here:
http://www.publicguardian.gov.uk/docs/LPAPA_No tes_1007.pdf

The personal welfare notes are here:
http://www.publicguardian.gov.uk/docs/LPAPW_No tes_1007.pdf

However, if you're missing the notes, there might be other paperwork you need as well. You can download complete packs, each containing 8 relevant documents from these links:

Property & affairs:
http://www.publicguardian.gov.uk/docs/lpa-crea tion-pack-property-and-affairs.zip

Personal welfare:
http://www.publicguardian.gov.uk/docs/lpa-crea tion-pack-personal-welfare.zip

Those downloads are zip files. You should save them to somewhere suitable on your PC (e.g. the desktop or 'My Documents') and then double-click (or right-click) the icon to 'unzip' them, so that you can see the 8 separate documents.
