I need to write an affidavit, are there any special forms ?
or do I just write a statement, what do I do next ?

You will have to swear an Affidavit in the presence of a Lawyer or Notary Public as it is a sworn statement of fact.

Thank you for your answer, but do I just write out what I need to say or is there a secial form ?
As you have to have a Solicitor swear it, you will probably find they will have it typed up for you.

An Affidavit is a document that could be used in Court without your presence.
Whoever has requested the Affidavit will have given the reason for the request......take the request to a solicitor (for him to draft the affidavit) & signed/witnessed in his presence.
fadeout...did you ever get round to writing your own affidavit or sworn statement? i need to write one to and was wondering as to the format, layout, wording etc.
