In laymans terms what exactly is it? Can you request information held about you and if so what information can you request and from whom?

It's the duty of the Government's Information Commissioner to ensure that both the DPA and the Freedom of Information Act are complied with. So everyone you need to know should be on the Information Commissioner's website. Here it is:
