It's suddenly dawned on me it's an elephant dress.  Elephant head with ear and, trunk down the arm!!

I agree Naomi, but that one is rather over the top!  A  Motsi Mabuse  type dress.  But Motsi has now changed to a much nicer style now completely! Since she had her boob job.
