How did Menma die in the anime Anohana The Flower We Saw That Day?

(I saw this anime a while ago, and I still can't get an answer. Her passing is such an important part of the story as everyone feels responsible for whatever happened that I think there should have been a flashback or something.

I believe she died in the river when they were perhaps swimming or playing tag or whatever.)

In case you didn't know, some of the locations in the anime are real places in Chichibu Japan.
