I have heard that a few people in the public eye have dared to utter this question.
The latest self-important miscreant being Miley Cyrus - apparently.
Does anyone know of any other cases?

Best answer answer for them is to say "Ask any of your friends (If you have any) they might be able to tell you.!!
Who is MIley Cyrus?
This happened to a colleague of mine when a self-important boss type asked her that question, in an attempt not to have to follow the same procedures as the rest of the people visiting the event. She turned to her colleague and said "I am really sorry but this gentlemen seems to have temporary amnesia, are you able to help him?" I wish I could think that quickly!
It was a typically busy, crowded, chaotic Friday before a holiday at the Atlanta International Airport. Travelers, friends, relatives, airport personnel, pilots, flight attendants, the handicapped and infirm being transported through the throngs of shoulder to shoulder people.

At one of the packed, ticket counters all of the ticket agents were doing their best to politely process each passenger as quickly as they could. A man toward the end of the snaking line of passengers was obviously impatient and very frustrated at having to wait so long in the slow moving line. He finally decided to march right up to the counter pulling his wheeled suitcase and demanded that he be given his boarding pass. The ticket agent turned, looked at him, blinked, took a shallow, deep breath and said, "Sir, as you can see there are many passengers ahead of you. We are doing our best to process the passengers as fast as we can. I'm afraid you'll have to get back in line". Outraged and red in the face, the man yelled at the ticket agent saying, "Do you know who I am ???!!!." The ticket agent turned, looked at him, blinked, took another shallow, deep breath, picked up the public address system microphone and said calmly, "There is a man at the Delta ticket counter who does not know who he is. Anyone who may be able to identify this man is asked to please step forward and identify him. Thank you".
Reminds me of a Bruce Forsythe anecdote! When visiting an old peoples home, he asked a resident Do you know who I am, ask Matron replied the old dear, she'll be able to help!
Terence Stamp, in the New Bond Street branch of RBS. About 1971.
my boss used to say it regularly, I wouldnt mind but he was a total tw@t
The late Simon Dee (remember him?). After his chat show fame dwindled, he fell on hard times, and was thrown off a bus for not having enough money for the fare. He flew into a fury and demanded "Do you know who I am?" Nobody did.

The best I experienced was a pompous garage owner in the town where I live, who thought of himself as a pillar of society. He accidentally drove into the back of my car one day, and when I got out and approached him he dismissively told me to "leave the car in and his men would fix it". I asked for his name and address, and where the car should be left in. "Don't you know who I am?" he said. "Don't you live in this town?"
Wasn't it our wonderful deputy P.M. who was confronted about using her Mobile Phone whilst at traffic lights, who actually asked 'Do you know who I am'
I think it was Oscar Wilde, who recounted the story of when he was at a dinner party, where one of the guests was prattling on about how important he felt he was, and after telling his tale of putting some dogsbody in his place said, "Of course, he didn't know who I was!". Wilde came back with, " And who were you, exactly?"
