fiat punto cust out

my fiat punto 60 will not start without having my foot on the accelerator , once i have reved the engine for a min then it settles down but the engine is very jerky (could be due to cold weather but also does this what ever time of day)
Even after driving about half an hour the problem does not go away it continues to be jerky only when not moving. Driving down the road it is fine, any ideas thanks
It has also cut out on me wile i have been coming up to junctions very dangerous
